Today (Thursday 12 March) is Social Prescribing Day, which Charlton Athletic Community Trust (CACT) is celebrating having been commissioned by Royal Borough of Greenwich Public Health and Wellbeing to deliver two aspects of Live Well Greenwich, the local Social Prescribing offer: the Live Well Line and Live Well Coaches.
Many things impact our Health and Wellbeing. Social prescribing enables GP surgeries, nurses and other primary care professionals to refer their patients/clients to a range of local services that can help provide support.
In Royal Greenwich, social prescribers are called Live Well Coaches and can offer up to six, 30-minute, face-to-face sessions with Greenwich residents, who they can support with a wide range of issues.
From employment and housing to healthy eating and exercise, there is a range of non-medical things that can have an impact on a person’s health. Live Well Coaches link local residents with the services that can help support them with whatever it is they might need.
